Medical Leaves of Absence
When time away from work is needed for any of the following reasons, employees should work directly with FSDS to determine if the time off qualifies for protection under The Family & Medical Leave Act.
- an employee’s own serious health condition
- to care for a family member (parent, child, or spouse) with a serious health condition
- for a child entering the home through birth, adoption, or foster care
- exigency leave arising out of the foreign deployment of the employee’s spouse, son, daughter, or parent
- To care for a covered service member with a serious injury or illness if the employee is the spouse, son, daughter, parent, or next of kin of the covered service member.
What employees need to do:
- If time off from work is needed for any of the reasons listed above, please inform your supervisor and he/she will use the FSDS Leave & Disability Consultation Form to contact our office.
- An employee can choose to contact FSDS directly prior to speaking with his/her supervisor. To do so, please submit a FSDS Leave & Disability Consultation Form and a FSDS staff member will contact you directly.
Workplace Accommodations
When an employee’s health is impacting their ability to perform their job, employees should work directly with FSDS to determine if a reasonable accommodation under the American’s with Disabilities may be appropriate.
What employees need to do:
- If an employee’s health is impacting their ability to perform their job, please inform your supervisor and he/she will use the FSDS Leave & Disability Consultation Form to contact our office.
- An employee can choose to contact FSDS directly prior to speaking with his/her supervisor. To do so, please submit a FSDS Leave & Disability Consultation Form and a FSDS staff member will contact you directly.
