This collaboration between Little Angels Learning Center and UI Family Services provides student parents with free, safe and quality child care during final exams to help them achieve academic success. Eligibility Criteria

The student parent has a child under age 13 years and the student is:

• Currently enrolled at least half-time or is involved in academic responsibilities equivalent to half-time at the University (exceptions may be possible)
• Is seeking a UI degree (undergraduate or graduate/professional) and resides with the child when the care is needed

Pre-Exam Child Care Dates

Dates and times:

• Fall 2020 dates coming soon!

Student parents are responsible for:

• Completing and submitting all forms listed below by the Wednesday prior to care
• Providing infant cereal and/or baby jar food and prepared formula/breast milk in bottles
• Canceling a reserved slot at a minimum of three days prior to scheduled services

Please note: If you do not cancel your reservation a minimum of three days prior to scheduled services, you could be denied from enrolling in future finals child care programs.

All forms must be submitted when requesting the reserved child care. Child care is provided on a first-come, first-served basis. Please print and complete all forms below:

• Exams Child Care Program Registration Form (pdf)
• Exams Child Care Emergency Contact Form (pdf)
• Exams Child Care Physical Examination Form (pdf) (You may provide your own record or use this form.)
• Certificate of Immunization (pdf) (You may provide your own record, but it must utilize the Iowa Department of Public Health Certification of Immunization form.)
• Exams Child Care Permission Form (pdf)

Care can only be provided on requested dates if completed forms are submitted.
