Frequently Asked Questions
Once I complete a Form W-4, will I ever need to complete another one?
Please check your W-4 withholding regularly. If your life situation changes, like getting married or having a child, it may effect how much tax is withheld from your paycheck. If you claim exempt on a W-4 form and you still qualify for an exemption, you need to fill out a new form by February 15 each year.
You can enter your information for an exemption from federal tax in Employee Self-Service. To claim an exemption from state tax, you need to fill out a paper form and provide it to Payroll Services. Paper forms can be found on the W-4 webpage.
I am a resident of another state but work in Iowa. Which state should the tax withholding be for?
You pay state taxes based on where you work. If you live in a state other than Iowa or Illinois but work in Iowa, fill out an Iowa W-4.
My paycheck is out of date (stale dated). What should I do?
To request a new paycheck, contact Accounts Payable at 319-335-0115. Banks do not have to cash checks that are six months or older.
How can I replace my lost or damaged paycheck?
If you lost or damaged your paycheck, contact Payroll Services to request a new one.

When are the payroll cutoffs?
Monthly payroll cutoffs are five business days from the end of the month. Complete your direct deposit and W-4 form requests before the cutoff in Employee Self-Service. You will receive your monthly paycheck on the first working day of each month.
HR representatives can submit departmental transaction forms any time during the month, but the forms must be completed through Workflow five business days from the end of the month.
Biweekly payroll is processed every other Thursday. To be included in the payroll, submit your time records by the Tuesday before. Departmental deadlines may vary. You will receive your biweekly paycheck every other Tuesday.
How can I access prior pay period earning statements?
To see and print your wage and earnings statements, go to Employee Self Service > Time & Pay > Paycheck History.
How can I receive my year-end tax form (Form W-2)?
University of Iowa W-2 forms are available in January of each year. Visit this page for additional information on receiving W-2 forms.
