This program offers you a variety of life insurance coverage levels and family options. If you would like to participate, you may select a coverage level between $100,000 and $1,000,000. Coverage is available in $100,000 increments.

Instructions for completing your enrollment into the AD&D plan
- Enrollment in this coverage is optional.
- If you do not want to participate in the Accidental Death and Dismemberment program, select option 90 (waive) from the drop-down.
- If you want coverage, select the option code from the drop-down corresponding to your coverage choice. The Option Code is the number in the parentheses located to the right of the price. Costs vary depending on the amount of coverage and whether you wish to insure only yourself or your family.
- Choose the recalculate function/button to refresh your current list of eligible dependents and beneficiaries if needed.
